Remote patient monitoring (RPM) can be used in a variety of ways to monitor many different types of people and for a range of purposes. From relatively short-term engagements to lifelong monitoring, there are many individuals who could benefit from the use of remote patient monitoring. The most popular use of this technology today is to monitor patients who have recently been discharged from the hospital and are struggling to manage a chronic illness. Frequently, these are individuals with COPD, diabetes, hypertension, high cholesterol, or obesity who are trying to avoid readmittance to the hospital. RPM gives patients the benefits of constant monitoring without being in the hospital or daily visits from a home care nurse. Caring for Aging Parents and a Missed Diagnosis

Life Changes Earlier this year, Laura arrived back in Michigan to care for her 89-year-old mother, Beth, and stepfather, Allen. Her stepfather, recently released from critical care at Hillsdale Community Home Care, and Beth with her own chronic health issues, became a full-time care job for Laura. Despite much support from home care nurses, physical therapists, and in-office physician visits, the care for both parents was overwhelming. Last month the Health Numeric team attended the Michigan Association for Home Care's 27th annual conference at the Grand Traverse Resort in Williamsburg, Michigan. The event drew over 240 attendees from around the state and a few from home health companies across the country. These influencial people in the health care field continue to make leaps in the industry, and we enjoyed hearing about their innovations and impact. “We take a whole year to plan this event,” said Barry Cargill, the executive director for Michigan Association for Home Care. The attendees included people of all different backgrounds including nurses, clinical supervisors, and sales and marketing members. Building on the opportunities we gained at last year's event, we are excited to collaborate with many of the groups and individuals serving the home care industry as the company expands. Reducing 30-day Hospital Readmissions for CHF Patients

The care I received with remote health monitoring technology was just as good as having a nurse come to my house,” stated a participant in Sharp HealthCare Lean Six Sigma project. The Sharp HealthCare Foundation conducted a study with a main focus of reducing 30 and 90-day readmissions through Remote Health Monitoring. The study targeted 80 Congestive Heart Failure patients, 66 of whom completed the 90-day program.  The study analyzed the outcomes of enrolled patients diagnosed with primary, secondary or tertiary heart failure in comparison to a reference group of similar patients, not enrolled in the RPM program. Remote Health Monitoring works to reduce complexity and increase efficiency of health care. RPM works directly into a patient’s home and is non invasive.
