Virtual Concierge Care is 24/7 access to care providers using telehealth, telemedicine, remote patient monitoring, and distant learning services as appropriate in a patient-centered medical home (PCMH) environment. A PCMH is a delivery model whereby you, the patient, receive treatment coordinated through your primary care physician to ensure that you receive the necessary care when and where you need it.

The VCC program also includes coordinated care services and ancillary supports to address identified conditions where you live, learn, work, play, worship, and age that affect your health and quality of life.
